The situation for Misato is a simple one. She gets really drunk at a bar and then has to deal with the consequences.

The situation for Asuka is that she had to go before being called in to pilot her eva for some reason (sync test or live combat)

I have three ideas for Karen from code geass. The first one is similar to the one above for Asuka where she has to pilot while needing the washroom.

The second one is an AU version where Lelouche abuses his Geass and uses it for his on perverse pleasures. This one can also work for shirley or any other girl in Code Geass except for C.C

Note: Spoiler warning
The third one after the events of episode 10 in the second season where Karen gets captured by the Chinese federation. Karen is held inside her Guren for a long time without being let out and she hasn't gone to the washroom since some time before Tizani's wedding so eventually she has to pee./

Oh, I'm glad you mentioned NGE, 'cause I'd love to see Asuka and/or Rei wet their plugsuits.   

For some reason, I've always wanted to see Relena Darlian from Gundam Wing get stopped short on the side of the road or something.  She's a diplomat for 9/10 of the series and in a private school for the other 1/10, so there's plenty of opportunity... it just never happens.  Oh well...  guess it lives on in my head ;)

Alright I have come up with at least two more scenarios for Karen from Code geass and I am working on a third one aswell.
Alright these first two both take place during episode 19? of the first season, its the one where zero, suzaku, euphy and karen get lost on the island.

#1) When suzaku finds Karen she is naked and taking a bath under the waterfall. Why would she need a bath? She has been out there as long as Zero, Euphy and Suzaku have and they don't take any baths. The reason is because when she woke up beside the waterfall she realized she had pissed herself so she takes her outfit off to wash it in the water and clean herself as well. 

#2) After Karen gets found by Suzaku he dresses her and then ties her up. At no point during the 24 hours does need to go, she can need to go pee again or go poo. Of course Suzaku wont take off her bindings unless she agrees to not try anything. Of course that will never happen so in the end Karen has to poop or piss her outfit or beg suzaku to let her go for a few seconds and of course Suzaku only agrees if he is aloud to make sure she doesn't try to escape.

#3) I have been trying to think of a way for Karen to pee her school uniform in public and I think I have a way for it to work. After becoming a black knight and working a lot at night Karen ends up sleeping during class a lot. On one occasion she falls asleep while needing the washroom and then she wakes up just as she starts to pee in the middle of class.

#4) This takes place before episode 1 of season 2 and after episode 25 of season 1. Karen has been working undercover at a casino as a bunny waitress. What happens when one of her clients picks her for some special services and what if this client is into desperation play? A Karen in a wet bunny girl suit that's what happens. Though this idea could be used  for any type of fetish. Buying the bunny girl Karen's services and the client has some sort of fetish that he partakes in with her help.

For those who don't know this is Karen

Any way I also have an idea for a Lucy FD. The scene starts out with Nyu being in control, Nyu is out walking along the beach when she needs to pee. She gets attacked by Bando again and hits her head causing Lucy to become the dominant personality. After fighting off Bando Lucy calms down and realizes too late that her body needs to pee. So, she ends up peeing her pants at the beach and has to walk back home with a wetstain on the crotch of her shorts.

Alright I have another character who I would like to see in an FD. The character is Tayuya of the sound four from naruto.

I would need like 6 pages to list all of the anime they had FD in...

Let's see...I've seen so many anime this list might end up being quite long. Truth is, there isn't a single anime I wouldn't like to see FD in

First up, Elfen Lied. Lucy/Nyuu already had one, but Nana and Mayu should've had one too.
Then, Naruto. Sakura and Ino should definitely have a scene.
Inuyasha. Kagome and Sango, of course.
One Piece, Nami
Bleach, Inoue
Lucky Star....don't even get me started-_-;;
School Rumble, all of the girls, especially Eri.

There's bound to be more, these were just the ones I thought of immediately

Yukari and Mitsuru from persona 3...that would be awesome @~@.
