I know this probably doesn´t helps AMERICAN Readers, but the German Movie `Mondscheintarif`(2001) has some good Female Desperation: The Main Character Cora gets a Bladder Infection and is shown suffering on a Graveyard in the Rain; after trying unsuccessfully to find out where the Toilets are, she runs off and is shown squatting behind a Gravestone.

Not an actual wetting or desperation scene, but in Pineapple express one of the girls said that the guy made her laugh so much she almost peed herself, and the guy was like i wish.

This isn't a from a movie, its from a TV series about a British pop band called S Club (7). I should know, i used to like them 

Its still a semi-good desp scene, takes a while to get to the actual scene in the video, but no wetting unfortunately.

And yes, when interviewed, the actress admits that it is a real wetting, and that the director wants the wetting scene to be authentic. She actually has to wet herself in front of the whole crew!

To expand on this (after having done some research, due to the lack of information given by qaw213):

Billie (Marion Cotillard) needs to use the bathroom while being interrogated by a federal agent, but he doesn't let her. He leaves the room and she wets herself. This can be heard but is very quiet, only one or two drops. He then returns and mockingly says, "Look what you did." Agent Purvis (Christian Bale) then carries her to the bathroom, and her dress is obviously wet.

Obviously as the movie is so new, there's not a clip on youtube, but it sounds like a pretty interesting scene.
